Job Description:

Position Type: Regular Scheduled Hours per 2 week Pay Period: 60 Primary Location: IA > WEST DES MOINES > MERCY WEST LAKES GENERAL SUMMARY: Assumes responsibility and accountability for assigned patients. Provides professional nursing care to these patients incorporating the nursing process, the environment, technical equipment, and health care members. Provides patient care activities to patients of diverse ages as defined below: Medical/Surgical Units Age 16 to death (adolescent, adult, geriatric) CCU/ICU Age 16 to death (adolescent, adult, geriatric) Mother-Baby Unit Women of childbearing years to death; newborns (neonate, adolescent, adult) Birthing Unit Women of childbearing years (adolescent, adult) Neonatal Intensive Care Unit Newborns to 6 months (neonate, infant) Variety Club CHC and PICU 0-18 years (neonate, infant, child, adolescent to age 18) Pediatrics 0-25 years (neonate, infant, child, adolescent, adult to 25) Post-Anesthesia Recovery 0-death (neonate, infant, child, adolescent, adult, geriatric) C/TSICU 0-death (neonate, infant, child, adolescent, adult, geriatric) Maternity Triage and Treatment Unit Women of childbearing years to death. ESSENTIAL FUNCTIONS : Administers nursing care to patients; includes assessment, planning, implementing, and evaluating the patients care plan in coordination with the physician. Provides patient/family or caregiver education and counseling in recognition and solution of physical, emotional, and environmental health problems. Supervises health care members (i.e., licensed practical nurses, patient care technicians, etc.) with patients. Utilizes computer applications and communication technologies appropriate to the work setting to enter, retrieve, and analyze information and to facilitate communication and education. When assigned to charge, makes patient assignments; triages patients; and problem solves with staff, physicians, patients, families, and ancillary departments. Maintains knowledge of documentation requirements needed for patients records. Works independently with limited direct supervision using the Nurse Practice Act, Mercy Medical Center polices, nursing standards, and professional judgement. Maintains knowledge of equipment set-up, maintenance, and use (i.e., IV pumps, monitoring equipment, drainage devices). Utilizes safeguards such as IV pump drug library and monitor alarms whenever available to maximize patient safety. Maintains confidentiality regarding all patient/personnel information. Works rotating shifts, weekends, and holidays as scheduled. Initiates emergency support measures (i.e., cardiopulmonary resuscitation, protecting patient from injury). Takes assigned call as defined in unit policy. Personnel Issues Attendance Works within the hospital expectations of attendance as defined in the Corporate Policy and the Employee Handbook. Nursing Process Assessment Collects data from patient/family to complete the Patient History and the complete assessment. Plan of Care Utilizes gathered data in determining priorities for patients physical, psychosocial, educational, and discharge needs; in formulating the plan of care; and in setting measurable patient-centered outcomes based on nursing diagnoses. Implementation Carries out nursing interventions safely and accurately utilizing knowledge, judgment, and skills to implement written physicians orders and the nursing plan of care. Evaluation Evaluates patient/family responses to implementation of the plan of care. Modifies/Revises plan of care based on these responses, progressing the patient toward expected outcomes. Documentation Performs documentation in the nursing unit and in the patient record accurately, concisely, and in a timely manner. Patient Education Identifies patient/family educational needs and implements individualized measures to meet those. Management/Leadership Coordination Demonstrates direction and coordination of health care members, patient care, and unit activities. Supervises delegated tasks and duties. Problem Identification/Problem Resolution Practices effective problem identification and resolution skills as a method of sound decision making. Professional Growth Demonstrates an awareness of self responsibility and accountability for own professional practice. Self Development Participates in self-directed learning and in continuing education to meet own professional development. Attends and successfully completes all generic and unit specific required education within the time frame defined in nursing policy. Research Participates in nursing research activities as requested. Technical Competency Demonstrates competence in the use of computers, including basic keyboard and mouse use, login functions, and Windows-based program use. Demonstrates the ability to access and use clinical information systems and electronic medical records to Assess, Plan, Implement, Evaluate and Document care as appropriate to th work setting. Demonstrates basic proficiency in the use of email and Internet. Demonstrates use of clinical databases to retrieve evidence-based practice guidelines and patient education materials. Interpersonal Skills Communication Utilizes communication devices effective to communicate with patients and care team members. Communicates all pertinent information concerning patient to appropriate health care members. Demonstrates effective and respectful communication skills including verbal, telephone, written and electronic communication, in all interactions with patients, families, and members of the interdisciplinary team. Work Relationships Formulates and uses effective working relationships with health care members, patients, and families. Promotes a cooperative atmosphere within the work unit. Hospital/Department Standards Staffing Works with personnel and staffing policies. Knowledge Base Assumes responsibility for knowledge of and adherence to approved hospital policy and nursing policies, procedures and protocols. Quality Management/Standards Participates in unit-based quality management and performance improvement activities as requested. Confidentiality Maintains confidentiality of patient, personnel, and institutional information. Patient Rights Demonstrates awareness and sensitivity to the rights of patient/family. Safety/Legal Requirements Functions with an awareness and application of safety issues as identified within the institution. Demonstrates awareness of legal issues in all aspects of patient care and unit function, and strives to manage situations in a reduced risk manner and promoting patient safety. Emergency Situations Performs efficiently in emergency situations, following established policies/standards. MARGINAL FUNCTIONS : Assists with patient care duties/tasks (i.e., emptying drainage devices, ambulating, hygiene. Assists with clerical duties (i.e., maintenance of chart, patient record, answering phones and call lights, processing physician orders). Maintains patient care supplies, equipment, and environment. Provides orientation/education for staff, students, and other hospital and community organizations as requested. Effectively assumes job responsibilities of ancillary support during absence.
