whose lives they touch.A Typical Day for a Performance Improvement
Partner with WesleyLife at Home/Home Health:
- Collects, compiles, and uses data to identify outcomes,
documentation, compliance, and performance trends.
- Collaborates with Clinical Manager-RNs, Clinical Educator, and
Director of Clinical Quality to identify areas requiring
performance improvement plans, additional education, or other
actions.
- Coordinates monthly QA meetings. Participates in case
conference activities and ensures conferences are conducted and
documented according to policy through attendance and record
reviews.
- Works collaboratively with Case Manager RNs to monitor
admission and discharge records for timeliness, appropriate levels
of care, completeness and accuracy of documentation, fulfillment of
physician orders, and completeness of medical records.
- Submits Outcome and Assessment Information Set (OASIS)
assessments to the Centers for Medicare and Medicaid Services (CMS)
upon completion. Reviews and works validation reports received for
each CMS submission.
- Participates in an on-call rotation; travels in all weather
conditions to all areas served by the department to perform client
visits as needed.
- Current Openings: Full-Time Monday-Friday with rotating on-call

- The pay range for this position is $75,025-$90,000 / year. The
starting pay rate will be based on years of experience.Mileage
