Job Description:
Mercy Home Care is seeking a full-time RN Case Manager! Join Our Team to help make a difference in people's lives while caring for patients in their own homes! Have the autonomy to create your own flexible schedule! We use EMR system, HomeCare HomeBase for time efficient documentation. Home Care Registered Nurse Responsibilities: Provides skilled care and education to patients and their caregivers Participates in the coordination of services with other disciplines and community resources as needed Evaluates and revises care plans/assessments based on changes in patients and/or their environments Participates in regular case conferences and in-services or special case orientations as needed and participates in on-call rotation Qualifications Graduate of an accredited school of nursing Currently licensed to practice in Iowa One year RN experience required Why Should You Join Our Team?
